Partnership Southwark are working with service users, carers and local communities to better join up services and support, tackle the causes of inequality and improve the health and wellbeing of Southwark residents.

People have told us that they want health, care and education services to be more joined-up, person centred and easy to access. We agree; services can often be too complex and fragmented, which leads to poorer health and wellbeing outcomes. By working together and with local communities, we can make sure that services are more consistent and responsive to the needs of service users, carers, and families, and we address the inequalities that exist within our communities; many of which have been exacerbated by the Covid-19 pandemic.

Our partnership brings together local health, care and voluntary organisations to:
