How social prescribing can help you improve your health and wellbeing
Social prescribing, sometimes called community referral helps you find ways to improve your health and wellbeing. It does this by connecting you to events in your local area. This could be anything from art classes and social activities for adults who are isolated, to getting benefits and legal advice. Social prescribing allows you to access activities that meet your wider emotional, physical and social needs.

Social prescribing services in Lewisham:
One Health Lewisham Social Prescribing Service
One Health Lewisham (OHP) is working with the Primary Care Networks to deliver a social prescribing service for people living in the borough of Lewisham or registered with a Lewisham GP.
A team of social prescribers works alongside Lewisham GPs to help residents find community opportunities and services to improve your wellbeing. Support is free and for up to 12 weeks
Examples of support include:
- Making appointments for housing, benefits and financial advice
- Healthy lifestyle advice and physical activity
- Joining and attending social activities and clubs
- Getting online and accessing digital support
- Befriending, counselling, bereavement and peer support referrals
- Finding employment, training and volunteering opportunities
- Completing Blue Badge/Taxicard/Dial-a-Ride applications
- Emotional support

Modality Lewisham
Modality Lewisham’s Wellbeing Co-ordinator Service provide a similar social prescribing service to that of OHL. It serves the patients of the following GP practices:
- The Jenner Practice
- South Lewisham Group Practice
- Bellingham Green Practice

Community Connections Lewisham (CCL)
CCL is a social prescribing service, supporting anyone living in the borough (aged 18 and over) who is looking for help to improve their health and wellbeing. They can help you to find out about local services and groups that are relevant to you and help you to access them more easily.
You can refer yourself, or they will accept referrals from professionals, family members or friends. If you are referring someone else, then please ensure you have received their consent first.
