One Bromley is committed to involving Bromley residents in how we provide joined up care for our residents.

As a member of the patient network, you can get involved as much or as little as you like.  Members have been involved in all kinds of ways. Including commenting on materials, joining workshops and focus groups, attending patient conferences and being involved in procuring new services.  

Here are some examples of how Bromley residents are influencing services.

FIBROMYALGIA PATHWAY REDESIGN – We are aiming to improve care for people with Fibromyalgia. We asked for views and met with the Bromley Fibromyalgia Group to shape our plans.

ONE BROMLEY PATIENT CONFERENCE – We hold patient conferences to inform and shape joined up working in Bromley. Our September event focused on improvements in GP practices and new ways of getting outpatient care.

ONE BROMLEY OUTPATIENT TRANSFORMATION SURVEY – We asked residents what they thought of our plans to provide outpatient services in different ways.

NEW GP SERVICE IN CARE HOMES – Care home residents were one of the groups who helped to shape a new GP service just for care home residents.

DEVELOPMENT OF THE ONE BROMLEY PROACTIVE CARE PATHWAY – How patients informed the development of Integrated Care Networks and the proactive care pathway. Patients on this pathway of care receive joined up care from a range of organisations. It is designed to support them and avoid any emergency hospital admissions.
